ADMISSION REQUIREMENTS

In order to register your child at ALIF Canada, the following documents and items are required:

1. Registration Form

2. Ontario Student Record (OSR) Consent Form

3. Proof of Birth (Birth Certificate/Citizenship Certificate or Card/Passport/Landing Documents)

4. Updated Immunization Record

5. Copy of Valid Health Card

6. Most recent Report Card (Kindergarten to Grade 12)

7. Credit History or Transcript (High School Students)

8. School Transportation Form & Policy  (if applicable)

9. Level of Performance in Entrance Assessment (if applicable)

10. Proof of Child Custody (if applicable)

11. $10.00 for 2 passport Size Photos for each child (will be taken at school)

12. Preauthorized Debit Form or a VOID Cheque

13. Administration Fees

14. First and Last Month Payment

15. Financial Agreement Signed by the Parent/Guardian

16. Signed Uniform Policy
